The records include details of any care or treatment you have received in hospitals or elsewhere in the NHS.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You have a right to request this information, and GP practices are obliged to show it to you.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">To get this information, please either speak to the practice team or use our online services.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you have previously registered for online services with your GP, you can simply log in and see your records, including details of previous appointments, medications, tests results and vaccines.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you haven\u2019t logged in previously, you will need to register an account to gain access. To do this, go to your GP&#8217;s online access page.<\/span><\/p>\n<p><strong>The same information is also available through the NHS App. You can find details of how to download, register, login and access your records on the NHS app here.<\/strong><\/p>\n<p><span style=\"font-weight: 400;\">You will also have a Summary Care Record (SCR) unless you have explicitly chosen not to have one.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">An SCR holds vital information about your medical history, medications, allergies and any previous bad reactions. It is available to anyone involved in your care and helps NHS staff make the best decisions for your health.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">SCRs are not currently available online, so please speak to the practice team if you would like to see yours.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"font-weight: 400;\">Information held by NHS Digital<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">NHS Digital also collects are stores patient information for <\/span><span style=\"font-weight: 400;\">research and planning purposes<\/span><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As with information held by your GP, you have a right to request this information and be shown it.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Often this information is kept in coded formats rather than readable text, but you will be given a list of what those codes mean.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">To get access to this coded statistical information, you will need to submit a <\/span><a href=\"https:\/\/digital.nhs.uk\/about-nhs-digital\/corporate-information-and-documents\/publication-scheme\/how-to-make-a-subject-access-request\"><span style=\"font-weight: 400;\">Subject Access Request<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">NHS Digital have a <\/span><a href=\"https:\/\/nhs-prod.global.ssl.fastly.net\/binaries\/content\/assets\/website-assets\/corporate-information\/subject-access-requests\/subject-access-request-application-form-april-2021-final.docx\"><span style=\"font-weight: 400;\">subject access form<\/span><\/a><span style=\"font-weight: 400;\"> that you can complete and submit by email to <\/span><a href=\"mailto:enquiries@nhsdigital.nhs.uk\"><span style=\"font-weight: 400;\">enquiries@nhsdigital.nhs.uk<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Please be aware you will need to supply various pieces of information and documents to prove your identity.
